Directions:

 

• Download the four forms and fax back Application and Consent Forms to 712-364-2234.

 

• You will need to fill out the Company Driver Job Application completely. For the safety performance history records request you will need to only fill out section 1, and we will need one of these for each previous employer.

 

• Fill out the Background Check Consent Form.

 

• We will need a copy of your license (front and back).

 

• We will need a copy of your DOT physical (short and long form).
